DJ-LE-285 Address Change
| Licensees are required to notify the Department of Justice Firearms Unit in writing within 30 days of changing their address. To do this you must submit a completed and signed address change form. Although not required, if you would like a new license to reflect your updated address you must include a check or money order payable to the Wisconsin Department of Justice in the amount of $12.

DJ-LE-285 Lost, Stolen, or Damaged Card
| If your CCW license is lost or severely damaged you may request a replacement license in writing by submitting a completed and signed replacement license form. There is a $12 fee for replacement licenses. Please include a check or money order payable to the Wisconsin Department of Justice along with your replacement license form and the damaged license, if possible.

DJ-LE-289 State of Wisconsin Out-of-State License Affirmation
| Form to affirm that a copy of a current or expired CCW license from another state has not been revoked for cause.

DJ-LE-290 Former Officer Concealed Carry Affirmation
| Section 175.49(3) of the Wisconsin Statutes permits the Wisconsin Department of Justice (“DOJ”), under specified conditions, to issue a firearm certification card to former federal and out-of-state law enforcement officers who reside in Wisconsin. Before issuing such a card, DOJ is required to verify certain information about the former federal or out-of-state officer, including the information requested on the front of this form.

DJ-LE-291 CCW Denial Appeal Form
| If your application for a CCW license was denied and you feel that this decision was incorrect you may appeal the decision through our appeals process for no additional charge. A copy of the appeals form was also included with your denial letter.

DJ-LE-293 Change of Name Notification
| Licensees are required to notify the Department of Justice Firearms Unit in writing within 30 days of legally changing their name. To do this you must submit a completed and signed copy of the name change form. There is a $22 fee associated with name changes. This fee is comprised of $10 for a new background check and $12 for a new license. Please include a check or money order payable to the Wisconsin Department of Justice for the background check and re-placement
license pursuant to Jus 17.08 (3).

DJ-LE-FH1 WI Firearms Dealer Registration Form
| Federally
registered firearms dealers must register with the Wisconsin Department of
Justice to sell handguns in Wisconsin. Firearms dealers use this form as their
registration application.

DJ-LE–286 Certification of Former Law Enforcement Officers
| Former
federal law enforcement officers and former law enforcement officers from other
states that now live in Wisconsin, can apply for their HR 218 certification
from the Wisconsin Department of Justice using this form. Former Wisconsin law
enforcement officers must apply to their former Wisconsin agency for their HR
218 certification.

DJ-LE–287 Concealed Weapon License Application
| Wisconsin
residents may use this form to apply for their Wisconsin CCW license using this
form and submitting to the Wisconsin Department of Justice along with
appropriate payment and proof of required training.

DJ-LE–287R Concealed Weapon License Renewal Application
| Wisconsin
residents may use this form to apply for the renewal of their Wisconsin CCW
license by submitting this form and appropriate payment to the Wisconsin
Department of Justice.
